Presence of large vessel abnormalities in patients with CADASIL is very unusual. To our knowledge, our patient is the first reported case of concomitant Vertebrobasilar dolichoectasia and basilar tip aneurysm in CADASIL, presented with acute infarction in posterior circulation territory.
